Ask enough to route an enquiry, without turning your first conversation into an application form.
Start with the decision the form supports
A contact form should tell your team who is asking, what they need and how to reply. A qualification form may also need company, team size or timeframe. Make each required field earn its place: if nobody uses an answer to route or prepare the enquiry, consider removing it.
Use a short question sequence
Begin with the business and the workflow, then ask for contact details. Helpful prompts include “Which process would you like to improve?”, “What slows the team down?” and “When would you like to begin?”. Use plain choices and an open text field for context the choices miss.
Design the confirmation around the actual result
Validate on the server as well as in the browser. Save the enquiry durably before showing success. If the CRM is temporarily unavailable, keep the enquiry queued and say it has been received. A confirmation should not claim that an email was delivered merely because the lead was saved.
Test the whole handoff
Test missing fields, an invalid email address, a double click and a temporary connection failure. Confirm that a retry reuses the same submission rather than generating duplicate leads.
